Coordinated School Health is partnering with the Marshall County Health Department to help our upcoming 7th graders receive the necessary vaccines to enter 7th grade next year.

A form will be sent home with your 6th grader from the Department of Health-giving consent for vaccinations. if you wish for your child to be vaccinated at school, please fill out and return the form, along with a copy of their insurance card and current immunization records, by May 2, 2023. Additional forms will be available in the school office or on the district website.
